In your first session, your Health Coordinator will get to know you—asking about your health, family, home, and daily life. As you go along, they’ll work with you to find the best support and solutions that fit your needs.
Our coordinators offer personalised help through one-on-one or group sessions, either in person or via telehealth, whichever suits you best.
By working with a Health Coordinator, you’ll have support every step of the way through your care and treatment—making your health journey easier and more comfortable.
How Your Health Coordinator Can Help:
- Plan and deliver support for psychosocial challenges and recovery, including setting goals with clients.
- Deliver health education and support community programs to boost mental and physical well-being.
- Coordinate health services by managing cases and connecting you with internal and external support resources.
- Provide evidence-based assessments and treatment plans, including mental health evaluations and regular reviews with other clinic professionals.
- Create personalised health care plans for DVA veterans and NDIS clients.
- Provide general and specialist counselling for emotional and mental health, with regular wellbeing check-ins.
- Provide individual and group counselling to reduce social isolation, boost health, and encourage shared support.
- Support and stand up for individuals and families, especially those facing social challenges, through personal assistance and community involvement.
